GUIDELINES FOR USE OF THE ON-LINE CITIZEN REPORT FORM:
The online citizen report form is intended to provide a convenient option for reporting minor thefts and damage to personal property.
To use this form all three of the following criteria must be met:
If your situation does not meet the above criteria, you are better served by having an officer meet with you directly to take your complaint. This will ensure that our Department is handling your incident in an efficient and effective manner. Contact the Chippewa Falls Police Department at 715-723-4424, (Press 1; to speak with the Dispatcher) to discuss your specific incident.
INSTRUCTIONS FOR FILING A CITIZEN REPORT FORM:
*Note: A case number will be assigned by the CFPD upon receipt of your report.*

Within three (3) business days you will receive a return email from the Chippewa Falls Police Department with the case number that has been assigned to your incident. This will serve as confirmation that your report has been received and is on file with our agency. The Patrol and Investigation Divisions will be provided with details regarding your report and an officer may contact you at a later date for additional information. In the event that an individual is arrested for involvement in your incident, you will be contacted with appropriate information.

- The incident in question must have occurred within the City limits of Chippewa Falls
- The total actual value of property taken and/or the repair/replacement costs of the Property damaged is $500.00 or less.
- The reporting party has no information about a suspect(s).

- Complainant information: Must be completed – All blocks are required.
- Business/Property Owner Information: Should be completed if applicable to your incident.
- Vehicle Information: Should be completed if applicable to your incident.
- Stolen and/or Damaged Property: This section required to itemize and describe, in as much detail as possible, the property stolen or damaged.
- Incident Information: A brief, but detailed description of the incident is required.
- Clicking the “Submit by E-mail” button on the upper right corner of the form means you have reviewed the report form and attest that all information and statements made therein are true and correct to the best of your knowledge.
- Clicking the “Submit by E-mail” button will electronically send your report to the Chippewa Falls Police Department.
